Death
Certificate: (860) 561-7430
A death certificate is on file in the Town Clerk’s Office in the
town where the death occurred and where the person lived prior
to his or
her death. A certified copy of a death certificate is $5. Click

Dog
Licenses: (860) 561-7430
All dogs over six months of age require a dog license. Applications for licenses
may be made at the Town Clerk's office.
Driveway Permits: (860) 561-7540
A Driveway Permit ($25 fee) is required whenever this type of work is done within
the street's right of way. In addition to the certificate of insurance needed for the permit,
a $10,000 Uniform Bond and a Liability & Indemnity
Form are needed (both forms available in the Engineering Office).
Excavation
Permit: (860) 561-7540
A Street Excavation Permit ($25 fee) is required whenever this type of work is done within
the street's right of way. In addition to the certificate of insurance needed for the permit,
a $10,000 Uniform Bond and a Liability & Indemnity
Form are needed (both forms available in the Engineering Office).
